10 High Impact Actions – implementing improvement in Primary Care

With funding available from NHS England it is a great opportunity to up-skill practice staff to deliver improved patient care.

We have designed a range of training programmes for receptionists, managers, GP’s and CCG/CEPN Primary Care Leads to put the 10 High Impact Actions into practice.

Have some budget left to spend this financial year? With half day programmes starting from £495.00 for 18 participants, you have a great opportunity to utilise left over funds! (cost dependent on location)

10 High Impact Actions – the interventions

Signposting

On completion of this programme, participants will be able to:

  • Appreciate the role of active signposting in general practice
  • Understand the need for appropriate patient care navigation
  • Understand the benefit of patients seeing the right person first time
  • Be aware of patient rights and any possible concerns

New Consultation Types

On completion of this programme, participants will be able to:

  • Understand the need for adding new ways of consulting
  • Identify practice stakeholders, their needs and expectations
  • Examine new ways of consulting
  • Identify possible risks and concerns

Reduce DNAs

On completion of this programme, participants will be able to:

  • Understand the need for adding new ways of consulting
  • Identify practice stakeholders, their needs and expectations
  • Examine new ways of consulting
  • Identify possible risks and concerns

Develop the team

On completion of this programme, participants will be able to:

  • Explored the benefits of working as a team
  • Fostered understanding and improved team working through enhanced communication
  • Explored new approaches to improve team performance now and for the future

Productive workflows

On completion of this programme, participants will:

  • Critically review service provision and particularly work flows
  • Examine potential efficiencies
  • Understand stakeholder needs more effectively
  • Apply a range of QI and workflow tools and techniques to service delivery in their own organisations
  • Implement the necessary changes within their teams
  • Handle the communication impacts from the changes

Personal productivity

On completion of this programme, participants will:

  • Critically review their own productivity
  • Matching their own productivity to patient’s expectations.
  • Consider the human and legal impacts of any changes proposed
  • Apply a range of productivity tools and techniques to their own workflow

Partnership working

On completion of this programme, participants will:

  • Understand the nature of collaborative working relationships within the health & public sector
  • Identify the essential features of partnership working
  • Plan and implement effective partnerships

Social perscribing

On completion of this programme, participants will:

  • Understand the concept of Social prescribing and its links to active signposting.
  • Identify the practice stakeholders and their emerging needs.
  • Consider how Social Prescribing can improve patient care.

Support self-care

On completion of this programme, participants will:

  • Understand how we can safely introduce higher proportion of patients self-caring and self-managing their conditions
  • Identify the practice stakeholders and their emerging needs.
  • Consider how this can improve patient care.

Develop QI expertise

On completion of this programme, participants will:

  • Critically review the service provision
  • Examine potential efficiencies
  • Understand stakeholder needs more effectively
  • Apply a range of QI tools and techniques to service delivery
  • Implement the necessary changes within their teams
